Dispute a Debit or Credit Card Transaction
If you need more information, think your statement or receipt is wrong, or have a merchant dispute, it is important to notify Fidelity.

If the total amount of your disputed transactions is $20.00 or less please email Elevations Credit Union directly, call 303.443.4672, or visit one of our branches. Otherwise, please follow the steps below:

  1. Complete the Dispute Form [PDF]
  2. Fax the form to Fidelity National Financial Services at 866.451.6263.
  3. Retain your confirmation that the fax went through successfully.  Please do not complete a form for pending transactions because we must verify that transactions have posted before opening a dispute. 

All disputes must be received by Fidelity within 60 days of the statement on which the disputed item appears. If your disputed transaction is past that timeframe you will need to resolve the matter directly with the merchant.

Possible Reasons for a Dispute:
  • You were double charged by a merchant
  • You cancelled the service but the merchant continues to charge you (please provide cancellation documentation with your dispute form)
  • You ordered a trial offer from this merchant and they continue to send merchandise to you and charge you (all merchandise must be returned to the merchant and you must retain proof of the return)
  • You paid by other means (you used another card or paid cash but the original charge still posted).
