Report Check Card Fraudulent Activity
As a member convenience, Elevations
fraud monitoring service monitors your account for potential fraud. However, if you have unauthorized charges on your check card this may be the result of a fraudulent transaction. Fraudulent transactions can often be a result of counterfeit or skimmed cards, computer breaches, or your card may have been lost or stolen and someone used it.
If the total amount of your fraudulent transactions is
$20.00 or less please
email us directly, call 303.443.4672, or visit one of our
branches.
When you have fraud on your check card, that card is blocked as stolen and a new card is ordered for you. You should receive the new card in 7-10 business days with the PIN to follow 3-4 business days later, or you can visit any of our
branches to get a card instantly issued.
A fraudulent transaction must be reported to Fidelity within 60 days of the statement on which the item appears. If your transaction is past that timeframe you will need to resolve the matter directly with the merchant.
First determine what type of fraudulent transaction you have: signature or PIN based. If you view your account information in online banking, a signature based transaction will be listed as "miscellaneous transaction." A PIN based transaction will be listed as "ATM withdrawal" or "Electronic purchase".
Signature Based Fraudulent Transactions
If you have determined that you have a signature based fraudulent transaction, call Fidelity's One Call Resolution number at:

OR follow the following steps:
- Complete the Lost or Stolen Cards request online.
- Complete the Fidelity Affidavit of Fraud [PDF] for any transactions that have actually posted to your account.
Please do not complete an affidavit for pending transactions because we must verify that transactions have posted before opening a case.
Note: Fidelity cannot see transactions that have posted to your account until the following business day.
- Fax the form to Fidelity National Financial Services at 866.451.6263 (Retain your confirmation that the fax went through successfully).
