Open an Individual Retirement Account (ESA) by Mail

Thank you for your interest in opening a Coverdell Education Savings Account (ESA) at Elevations Credit Union. We would be happy to assist you complete the applications to establish a new ESA. Please feel free to call 303.443.4672 or 800.429.7626, or stop by any branch location (not available at CU Service Centers).

Non-Members:

  1. Review the Membership Avenues & Membership and Account Agreement [PDF]
  2. Complete the Membership Application [PDF]
    (Select the Savings and the ESA account types)
  3. Complete the appropriate form: Coverdell ESA Application [PDF]
    1. Where the form says "I instruct the credit union to invest this ESA in the following investment" or “ESA Investment Chosen,” indicate your choice from below:
      1. ESA Savings opening balance $100.00
      2. ESACertificate with minimum and opening balance of $100 (indicate 3, 6, 12, 18, 24, 36, 48 or 60-month term)
    2. List your beneficiary information.
    3. If you have designated your spouse as less than 50% primary beneficiary, your spouse will need to sign for consent.
    4. Retain a copy of the Agreement and Disclosure information for your records.

Complete Application Should Contain:
  1. Membership and ESA Applications with Account Owner's Signature and date
  2. Copy of your photo ID (unexpired driver’s license, military ID, or passport)
  3. Savings deposit of $25 or more

To arrange a direct transfer or direct rollover from an ESA at another financial institution or a 401k, follow these simple instructions.
Members:

  1. Review the Membership and Account Agreement [PDF]
  2. Complete the Membership Application [PDF] to establish your ESAaccount
    (Select the Savings and the ESA account types)
  3. Complete the appropriate form: Coverdell ESA Application [PDF]
