Thank you for opening an ESA. Please complete the following steps in order to ensure your ESA is established accurately and efficiently:

Non-Members: Please complete the New Membership and Services Application [PDF] for the child to open a savings account with $25 or more (establishes the child’s membership), and a Coverdell ESA (write “ESA” in the “Other” blank under the Account Type section). Then complete the Coverdell ESA Trust Application below, leaving your account number blank.

Members:
Complete the appropriate form: Coverdell ESA Trust Application [PDF]

• Where the form says "I instruct the credit union to invest this Coverdell ESA in the following investment" indicate your choice from below:
  • ESA Savings with opening balance of $100
  • ESA Certificate with minimum and opening balance of $100 (indicate 3, 6, 12, 18, 24, 36, 48 or 60-month term)
• Sign and date under Grantor’s Signature. Retain the Agreement and Disclosure information for your records.
