Setup a Simplified Employee Pension Plan by Mail
Thank you for your interest in setting up an SEP at Elevations Credit Union. Please complete the following steps in order to ensure your SEP is established accurately and efficiently:
  1. Complete the Credit Union SEP Contribution Agreement [PDF]

  2. Bring your completed form to any Elevations branch location or mail it to the following address:

  3. Instruct your employees to open an Elevations Traditional IRA (or open an IRA yourself if you are self employed).
