Request a Name Change

To request a name change, please follow the below steps:

Fill out a new Membership and Services Application (PDF). Complete and sign the Membership and Services Application with your NEW name. (If you would like to order a new Visa Debit Card or checks in your new name, you can designate that in the same form.)

Photo copy one of the following document options supporting your name change:
A copy of your marriage license and updated social security card with the new name showing on it.
A copy of your unexpired government-issued ID stating the name change.
A copy of the court document stating the name change.

Mail or Drop off your documentation.
Stop by a branch and deliver your application to one of our Financial Solutions Guides, or, mail the Membership and Services Application, along with photo copies of your supporting documents, to:
